If I buy the app for .99, do I have to sign up for the gold membership in order to reserve movies online? A lot of the negative reviews say you have to have the gold membership.

No, this is not true. The only thing a Gold Membership is required for is to use the Movie Queue feature, just like on this website. You can definitely reserve movies and use all other features of the app without any membership at all.
Thanks for the question, I appreciate the opportunity to clarify how it works. I really wish they would allow developer responses in the app store reviews, that would really help a lot in so many ways!
